Household Information

If you have any phone numbers that are not German, and do not use WhatsApp, you will need at least one parent/guardian and one local emergency contact that does have a German number, or uses WhatsApp so we can ensure we can contact someone easily.
Please include a phone# and name for each of your local Emergency Contacts.
Please include the names of everyone who has permission to sign out and pick up your child (Mom, Dad, Siblings, Neighbor, Family Friends, etc.). If you are giving permission for a sibling to sign out and pick up younger siblings, they must be 12 years or older.

Student Information

Register all Students first and then Volunteers at the end. You can also add other students/adults after submission. Change to Leader/Volunteer if you're an adult registering to help with classes or if you are registering a teen (who has already been Confirmed) who will be serving as a Junior Catechist. If you are seeing incorrect boxes to fill, just click again on the drop down and it will refresh.

Medical and Program Release: As a parent/guardian, I give my permission for the above minor to attend Religious Education activities including regular classes and any special activities from September 2025 to August 2026. I authorize treatment under the direction of any licensed physician of the above minor in the event of a medical emergency which in the opinion of the attending physician may endanger his or her life, cause disfigurement, physical impairment or undue discomfort if delayed. This authority is granted after reasonable effort has been made to reach me by phone. I will not hold the church, or their staff, administration, or workers, liable for any injury to or loss of possessions by the above minor during any activity either on the church property or away, including regular meetings as well as special events.

Photo Release: Photographs are sometimes taken of ministry activities for publicity and promotional purposes, which include, but are not limited to, in-house presentations, church web site, brochures and bulletins. By signing below, you are granting the church to use photographs of the above mentioned minor(s) as stated.

Drop-off and Pick-up Policy: 1. Parents will escort their children to the Ramstein Intermediate School Cafeteria before 10:30 am. 2. Parents will take their child to the Catechist and sign their child in on the clipboard that has the Class Attendance Sheet. You may not leave until the adult Catechist or Aide is present. 3. Catechists will escort their class to the classroom any time after 10:35 am. 4. Latecomers will need to be escorted to their classroom so the parent can sign them in. 5. At 11:55, classes will be escorted back to the cafeteria and parents must sign their child out before leaving. Friendly Reminders: *Someone who you have given permission MUST BE PRESENT to escort your child off campus* *Please be prompt in picking your child up after class! Class is over at 1200*

Is your child 12 years old or older?: If your child is 12 years old or older, they are allowed, with your permission to sign themselves out at the end of class. This can be useful so you don't have to come in each time and/or they don't have to wait on you. Many also like to allow this so they can go sign out younger siblings. If you do give permission, you are responsible for ensuring your child knows your directions for where they are to go after class. They may be able to walk home if you live on base, or they should know where to meet you for pick up after each class. If you do not give permission, your child will stay with their teacher, and you will need to come sign them out as usual.
